As a Minimum Data Set (MDS) Nurse, you will be essential in ensuring the accuracy and completeness of the MDS assessments which are pivotal in outlining holistic and tailored care plans for our residents. This involves thorough data collection, collaboration with healthcare teams, and adherence to state and federal guidelines to ensure optimized care and wellness outcomes for each individual. Conduct comprehensive and systematic assessments of patients' clinical, functional, and psychosocial baselines using the MDS. Coordinate with interdisciplinary teams to formulate individualized care plans. Ensure compliance with federal and state regulations concerning resident assessments and care planning. Monitor and update residents' care plans based on evaluations and changing health statuses. Provide training and support to nursing staff regarding MDS processes and updates. Participate in quality assurance procedures to improve patient outcomes. Maintain accurate records and documentation in accordance with legal requirements. Strong analytical and problem-solving skills Excellent communication and interpersonal skills Detail-oriented with a high level of accuracy Experience in clinical assessments and resident care planning Ability to work collaboratively in an interdisciplinary team Knowledge of state and federal healthcare regulations Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current Registered Nurse (RN) license in the state of practice Certified MDS Coordinator (CMDC) certification preferred
