agency for you. SUMMARY DESCRIPTION OF PROGRAM Heartland Bridges
houses two programs, Bridges Transitional Living and Bridges Crisis
Care. Transitional Living is a short-term transitional housing
program enriched with supportive services designed to assist people
with a diagnosed mental health or co-occurring condition while they
transition to stability in the community. Crisis Care is a
residential stabilization service designed for individuals who need
a safe, secure environment for three to five days while stabilizing
from a crisis before returning to the community where they live.
SUMMARY DESCRIPTION OF WORK The Case Manager III provides intensive
case management services to an assigned caseload of Heartland
Bridges clients. This role coordinates supportive services designed
to meet client needs while working with them to develop a plan for
wellness, recovery, and stabilization. Strong decision-making,
problem-solving, and interpersonal skills are essential.
Compensation: between $19.74 and $23.54 per hour (wage is
determined by total years of relevant experience) Work Hours: 40

company perks MINIMUM QUALIFICATIONS Associate degree in human
services field (including but not limited to, psychology, social
work, mental health counseling, marriage and family therapy, or a
related field), Bachelors preferred Two years of experience in
behavioral or mental health services One year of experience working
with adults with serious mental illness or substance use preferred
Valid driver’s license and acceptable driving record Essential
Duties and Responsibilities Respond to crisis care referral calls,
assess individuals by gathering information, and provide
appropriate and accurate application verdicts while maintaining
strict confidentiality. Complete enrollment/intake for eligible
transitional and crisis clients. Assist Transitional Living clients
in identifying strengths, needs, and goals to develop
individualized service plans. Provide support services to Crisis
Care clients, including assistance with developing and revising
safety and stabilization plans, and connecting clients to medical,
therapeutic, or other identified providers. Document client
activity, service plan progress, and needs according to established
record-keeping procedures. Provide problem-solving and crisis
intervention services to empower clients toward self-sufficiency.
Screen for mental health and substance use treatment needs and
provide linkages to appropriate services. Assist clients in
locating, obtaining, and retaining suitable housing through housing
search and placement services. Develop supportive relationships
with clients and respond to their needs during shifts. Ensure
required information is collected, entered in prescribed formats,
and reported in a timely manner. Provide program information and
referral services to clients. Monitor client self-administration of
medication and follow recording procedures. Obtain and maintain
certification to administer non-parenteral medications in the state
of Iowa. Accurately record client outcomes in designated databases.
Provide psychoeducation on life, social, and coping skills; meet
with clients for weekly Life Skills Inventory coaching sessions.
Demonstrate knowledge of current community and HFS programs and
resources to meet client needs, including mainstream benefits,
income support services, and Integrated Health Home services.
Utilize trauma-informed care, person-centered, and culturally
competent principles in providing services to clients. Evaluate the
efficiency and effectiveness of community service providers to
ensure clients receive quality care. Orient clients to Bridges
protocols and address client non-compliance as necessary to
maintain a safe and secure environment for residents and staff.
Conduct client room safety checks and searches per program
protocol. Maintain and support upkeep of the facility. Work
independently with minimal supervision and collaboratively as part
of a team. Display a courteous and caring attitude to clients,
volunteers, and visitors of the agency. Adhere to all specific
program and agency procedures, policies, and requirements.
Collaborate with program area staff, volunteers, and other agency
staff. Develop personal and program-related skills through
participation in internal and external training opportunities,
including printed material and audio/visual media. Create,
maintain, and share, as appropriate, a dynamic self-care plan.
Perform essential functions of this job on the company’s physical
work site. Perform other program-related duties as assigned.
