Good Samaritan Health Center is actively seeking dedicated LPN or RN professionals to join our Long Term Care (LTC) team in addressing the unique needs of our community by delivering exceptional and compassionate nursing care. With a mission centered around advancing holistic health and wellness outcomes, our LTC nurses play a critical role in enhancing the lives of our patients through personalized, patient-centric healthcare services. Candidates will immerse in a supportive environment aimed at promoting the health and well-being of underserved communities while growing professionally through robust training and development programs. Administer prescribed medications and monitor patient response. Assist with daily living activities and ensure patient comfort at all times. Collaborate with a multidisciplinary team to develop and implement comprehensive care plans. Maintain accurate patient records and report any significant changes in condition. Educate patients and their families on health preservation and disease prevention. Implement infection control protocols and participate in quality improvement initiatives. Strong clinical skills in a long-term care setting. Excellent patient assessment and monitoring abilities. Proficient in the use of medical software for record keeping. Strong communication and interpersonal skills. Ability to work effectively in a multidisciplinary team.
