Our RN Care Managers play a critical role in enhancing patient outcomes by managing and coordinating comprehensive, patient-centered care across different settings. This role demands a combination of clinical expertise, compassionate patient engagement, and meticulous case management. Assess patient needs, develop, implement and coordinate individualized care plans. Collaborate with other healthcare professionals to ensure seamless delivery of care. Provide education and support to patients and their families on health conditions and wellness management. Monitor patient progress and adjust care plans as needed. Ensure compliance with healthcare regulations and standards. Document all patient interactions and interventions in their medical records. Strong clinical skills in a healthcare setting Excellence in communication, both verbal and written Proficient in care planning and case management Ability to make critical decisions under pressure Experienced with EMR (Electronic Medical Records) systems Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current Registered Nurse (RN) license valid in the state of employing facility Certification in Case Management (CCM) preferred but not required
