In this role, you will be instrumental in enhancing the quality of patient care through effective management and coordination of patient care plans. This position requires a compassionate professional who can successfully administer and organize all phases of the health care provision to our diverse patient population. Assess, plan, implement and evaluate patient care plans in a collaborative healthcare environment. Coordinate with healthcare team members to facilitate integrated and comprehensive care. Participate in patient and family counseling and education. Ensure compliance with healthcare regulations and standards. Manage resources efficiently to ensure quality of care. Track patient outcomes and suggest improvements to care plans based on patient interactions and available data. Strong clinical skills with a focus on patient care and management. Excellent communication and interpersonal skills. Ability to work collaboratively in a multidisciplinary team. Proficient in healthcare software and systems. Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current Registered Nurse (RN) license. Certified Case Manager (CCM) is a plus.
