Summary: The ACT Nurse is a core member of The Well’s Assertive
Community Treatment (ACT) team, providing nursing care and
integrated support to adults with serious mental illness (SMI).
This role manages the medication system, administers and documents
treatment, monitors health conditions and side effects, coordinates
with outside providers, and promotes health education and
prevention. The ACT Nurse also provides training to teammates on
psychiatric symptoms and medications, and partners with clients to
develop creative strategies for medication adherence. Because ACT
is an interdisciplinary model, the Nurse must be highly
collaborative, enjoying work alongside counselors, peer
specialists, case managers, prescribers, and other staff. The ACT
Nurse values the strengths of each discipline and sees the team
itself as the treatment, ensuring that clients benefit from the
full range of expertise the model offers. Until the ACT caseload
grows to full fidelity, the Nurse may also assist with medication
management for clients seeing The Well’s psychiatric provider and
support medication-related needs in residential programs. This is a
full-time, in-person position based in Norfolk, NE, with frequent
community travel to meet clients where they are. Core Values: ·

Qualifications Education & Credentials Licensed RN in Nebraska. At
least one year working with adults with SMI. Clinical Skills
Proficient in psychiatric nursing, medication administration, and
side-effect monitoring. Familiar with screening tools and metabolic
syndrome protocols. Knowledge of co-occurring conditions and
trauma-informed, recovery-oriented care. Administrative Skills
Accurate in eMAR and EHR documentation. Knowledge of HIPAA, 42 CFR
Part 2, and ethical nursing standards. Professional Attributes
Strong communication, teamwork, and problem-solving skills.
Flexible, client-centered, and creative. Willing to adjust schedule
for client needs. Key Responsibilities 1. Medication Management
Manage all medication processes (ordering, refilling, packaging,
storing, distributing). Administer oral and injectable medications;
maintain injection schedules. Document services accurately and
monitor for side effects. 2. Health Screening & Monitoring Perform
assessments, vital signs, and lab tracking for metabolic syndrome.
Monitor chronic conditions and preventive care needs. 3. Client
Support & Education Develop individualized medication adherence
strategies. Provide health education and support self-management.
Accompany clients to appointments as needed. 4. Team Collaboration
& Training Participate in daily meetings; share nursing insights.
Train staff on medications and psychiatric symptoms. Coordinate
care with internal and external providers. 5. Program Compliance &
Quality Ensure documentation meets standards. Engage in quality
improvement, maintain licensure, and complete required training.
Support medication needs in other programs as assigned. Physical
Requirements Frequent travel and community-based work. Regular
sitting, standing, walking, and computer use. Occasionally lift up
to 25 lbs. Must be adaptable to various environments.
