Job Description:
Work Shift: Primarily days with night, weekend and holiday
on-call involvement (United States of America) The pay range for
this position is listed below. May be eligible for a $10,000 sign

Responsible for utilizing the nursing process to provide nursing
care for Avera@Home patients following the ANA Standards of
Clinical Nursing Practice. The Nurse assumes responsibility for the
outcome of nursing care provided to patients in their homes or in
facilities. The Nurse demonstrates the knowledge and skills
necessary to provide exceptional care for the population served.
Collects patient health data. Analyzes the assessment data in
determining nursing diagnoses/problem focus. Develops a plan of
care that prescribes interventions to attain expected outcomes.
Implements the interventions identified in the plan of care.
Systemically evaluates the quality and effectiveness of nursing
practice. Collaborates with the patient, significant others, and
healthcare providers in providing patient care. Considers factors
related to safety, effectiveness, cost in planning, and delivering
patient care. To perform this job successfully, an individual must
be able to perform each essential job function satisfactorily
including having visual acuity adequate to perform position duties
and the ability to communicate effectively with others, hear,
understand and distinguish speech and other sounds. Reasonable

Required Education, License/Certification, or Work Experience:
Drivers License - Licensing Board This position requires a valid
driver's license and that the employee is insurable by the Avera
Health automobile liability insurance carrier Upon Hire Registered
Nurse (RN) - Board of Nursing An active license in the state of
practice. Preferred Education, License/Certification, or Work
Experience: ~ Minimum two (2) years related experience and/or
training or equivalent combination of education and experience.
