The Health Center Association of Nebraska (HCAN) is actively
recruiting a Primary Care Physician to join a Federally Qualified
Health Center (FQHC) in Omaha, Nebraska, where you will provide
high-quality, patient-centered medical care for all. Utilizing an
integrated care approach, our providers work collaboratively to
address all aspects of patients health and believe that
exceptional, compassionate care should be accessible and affordable
for all. Medical, dental, behavioral health, and outreach services
are coordinated to ensure well-rounded, comprehensive care.

Your Role:
- Deliver primary medical care to a wide variety patients
- Accurately chart and document patient visits in a timely manner
using the EHR
- Participate in quality improvement initiatives to improve
clinical and operational processes
- Counsel patients about health and illness and discuss
preventive health measures
- Participate in after-hours and weekend coverage as directed by
Chief Medical Officer
- Collaborate with an interdisciplinary team to improve health
outcomes
What We Are Looking For:
- Graduate of an accredited school of medicine with an
unrestricted license to practice medicine in the state of Nebraska;
Federal DEA License
- Board-certified or board-eligible and working toward becoming
certified
- Knowledge of PCMH model of care, FQHC healthcare delivery
model, and public health principles
- Demonstrated professional commitment to providing services to
medically under-served individuals in a public health setting
- Ability to foster strong patient-provider relationships
