*Overview*\nSince our doors opened in 1968 CHI Health Plainview has
been providing patients of Pierce Antelope and Knox counties in
Nebraska with exceptional care and quality outcomes. We are a
15-bed critical access facility with inpatient and outpatient
services.\n \nThe Licensed Practical Nurse (LPN), within their
scope of practice and under direction and supervision of a
Registered Nurse or Licensed Practitioner is responsible for
contributing to the assessment, planning, implementation and
evaluation of nursing care to a designated group of healthcare
consumers across the continuum. Provides services which encompass
the spiritual, physiological, social, and psychological aspects of
care of the sick, prevention of disease, or conservation of health.
The position contributes to the mission and core values of CHI
Health.\n\n*Responsibilities*\n \n \n \nCHI Health will supplement
weekend staffing by offering its employees a "Weekend Option"
program in which clinical (patient facing) employees in covered
departments have the ability to work more weekend shifts for
increased compensation. Weekend agreements will be 6 months in
duration and be renewable in July/January following initial
enrollment.\n \n\n* Under the direction and supervision of the
Registered Nurse or Licensed Practitioner follows the nursing
process which includes: assess, plan, implement, and evaluate. The
nursing process is an ongoing process that can end at any stage.
\n* Assessment - Demonstrates a systematic, dynamic way to collect
and analyze the following healthcare consumer data including:
psychological, physiological, sociocultural, spiritual, economic
and lifestyle factors. Contributes to the RN initial assessment and
may perform re-assessment. \n* Plan - Sets measurable and
achievable short and long term goals according to standards of care
based on the assessment by the Registered Nurse in collaboration
with the healthcare consumer and as part of the multidisciplinary
team. Participates in the development and modification of the plan
of care. \n* Implement - Implements and/or modifies the plan of
care using assessment data, clinical reasoning based upon standards
of care and evidence based practice. This includes care
coordination, education and discharge planning. The nurse is
accountable for the care directly, indirectly or through
delegation. Professional and legal standards for delegation must be
followed. \n* Evaluate - Uses a systematic ongoing process to
evaluate progress towards identified healthcare consumer goals.
Revision of plan of care when needed involving the healthcare
consumer and the multidisciplinary team. \n* Documents all phases
of the nursing process in the healthcare consumer's record. \n*
Demonstrates personal and professional responsibility for ongoing
development, practice, and education. Maintains a safe work
environment. Fosters a collaborative environment in support of the
nursing profession.\n \n \n\n*Qualifications*\nRequired:\nGraduate
of a LPN nursing program \nActive LPN licensure in the state of
Nebraskaor nurse licensure compact.\nBasic life support (BLS).
Acceptable credentialing bodies and certifications include the
following: American Heart Association: Basic Life Support.\nIf
hired in a NE facility, approved LPN-IV therapy course required if
LPN program completed outside of NE or in Nebraska completed prior
to 2016 or validation of prior LPN-C required. \n \n--- Every
Weekend (Option A)\na. Full or part time (FTE 0.5 or greater)
employees\nb. Works every weekend (minimum 24 hours or 2 complete
shifts; Friday night shift, Saturday day/night shift, and Sunday
day/night shift)\nc. Night shift employees are eligible for the
weekend option differential for a 3rd weekend shift\nd. Eligible to
work above their weekend commitment based on department
needs\n\n*Pay Range*\n$18.74 - $25.77 /hour
